This is a list of dropped Activators (Avatars) I’ve come across so far. Please feel free to add to the list.

Duhjalm Sentinel Activator (Fabled) (from every group of 2 or more duhjalm in Kraytok's Fortress-easy mode

Empowered Terror Activator (Legendary) (found in Halls of the Forsaken)
Libant Frostfuser Activator (legendary) Very, very rare drop in CM.
Loaminess Keeper Activator (Legendary) (off the Construct of War in Temple of Rallos Zek)
Nightblood Reaver Activator (found in Necrotic Asylum)
Strifewing Stalker Activator (Legendary) (from several random aviak mobs in Palace of the Awakened)
Windrider Curator (healer, drops from any Valkyrie/Windrider type mob like those in Everfrost)

We got a Loaminess Keeper Activator (Legendary) off the Construct of War in Temple of Rallos Zek: Foundations of Stone (EM) tonight.

Libant Frostfuser Activator (legendary; vampire mage; essentially same abilities as starter mage) Very, very rare drop in CM.

Windrider Curator (healer), drops from any Valkyrie/Windrider type mob like those in Everfrost

I don't remember its name, but I got a legendary treant activator off a corrupt treant in Greater Faydark.

Loaminess Keeper Acticator (Legendary) - Had this drop off the Treasure Guardian - spawned mob during 'To the Ilse of Fear' quest in Tenebrous Tangle

I got this off a vampire in Mistmoore Catacombs. It was a body drop from a grey mob.

Duhjalm Sentinel Activator (Fabled) also drops from the Dhujalm deforesters in EW.

Activators can come from any mob of the same type, just like the spawners do. From my experience, the harder the mob is the more likely to drop the rarer spawners and activators, just like real loot is. I can confirm even low level ettins drop the Duhjalm Sentinel, but in Kraytocks they are a dime a dozen (drop from pretty much every trash encounter). The Libant Frostfuser, i got 6 from one clear of the castle (1 from the basement in Catacombs, 2 from the first floor and 3 from the epics on second floor). Also got the Empowered Terror Activator from a normal mob somewhere in the world (i dont remember what zone, but i have not run any instances ).

Faro'Nuk Marauder Activator (Treasured, Heirloom) Looted it from the corpse of a tormented servant in Lockjaw's Lair.

Treespirit Earthcaller Activator dropped from the wood elementals that are roaming with the treants near the Irontoe Brigade camp in BBM.
