Recently I've made a few changes, including taking the menu choices off of hotbars, adding a new target box, and a smaller group window.  I'll post an updated on later on today.

Clean, not too cluttered. I don't keep all my hotkey pages up.. I just flick through them when I need a spell or macro from a different page. I'm used to doing that already with EQ1, so it comes as second nature to me. =)

I like everything in one place which I suppose you can see. This setup also allows a good deal of view left, so even tho it looks cluttered its managable. Dunno, I have an odd sense of organization. Only thing I dont like about my UI is that my monitor caps at 1280x960...could look so much better in a higher res.
Everything is the same transparency with and without mouseover. Its just easier to me. Seems like you should either have it transparent or not, dont see a use in mouseover differences. Also, the smallest chat box on the far right side isnt just empty - its for my server trading group...it wasnt active when I took the screenie.

Here is mine.  I like some solid lines/borders for my interface, so I try to balance it with the clear.

I am holding off on tweaking my UI a lot until my new system comes, and I know what res I can run it on. Some great ideas here though.

Here is mine. Since I'm a healer I do not need the combat screen, I can see the damage flying over the mob's head just fine. I play on 1280x widescreen mode. You can get all the mods that I used from http://www.eq2interface.com/downloads/index.php? SMILEY

This is my UI setup. I am really crazy about seperating my chat into various windows. I also like to keep my eyes on the bottom of the screen.
Therefore my hotkeys, group window, target, and debuffs, are located there next to my battle and group chat.
 
Bottom.
1) My hits/miss, My Spells/fizzle/wore off, Mob hit me, Hostile spell hit/miss me, (crits, warnings etc.)
2) Group, Say, Shout, Raid, Status, Tells, etc.
 
Top.
1) Guild.
2) NPC tells, /who list, merchant purchase, ooc, auc, etc.
3) Others spells.
4) Others hits, mob missing me.
 
And thats why the chat bug that resets where my chat goes really ticks me off! hehe.

Target shows up in the blank spot next to my Hps. Here's to hoping HTML works cause it's the only way I can think to add pics here:Here's the link if it won't work anyways.I spent a lot of time customizing colors ... Actually over an hour on the colors alone. Then EQ2 reset all my colors cause it was cranky or something. You can click through the chat boxes and right click on the frames to get at the options. Still requires more mods. I'm waiting on the awesome people at eq2interface.com to make more. I've tried messing with em but it's frigin confusing to mod UI stuff. Most I was able to do was add the zone name to this UI. Which I stole the coding from someone elses UI to figure it out.

I tend to like umm a certain neatness to my UIs. This is it. (slow server, forgive me)
 
Damage, spell casts, worn off, "battle spams", exp etc are all under the Combat window
Grp, gu, ooc, shout under the Chat window
NPC spams and removing spells from character spams are on the middle left window

This is my new GUI.  It's largely based on the needs of the Conjuror class -- everything is pretty much in one corner so I can micro all those DoTs and stuns during battle SMILEY

On my interface I like everything being squared off and within bounds. One chat window being taller then the other and things just looks awkward imho. However before I took this screenshot i must of accidently dragged one bigger then other. Imagine them the same height!Simply, works on 1024x768, and doesn't require modding.

Modifications and Layout -  
1/2 sized Hotbars in box array with room to grow (72 in all)
Chat Windows without scroll bars.
Left Chat windows for all items including battle spam.
Right Chat window for group, guild, and spell wearoff warning.
Spell wear off warning color set to bright yellow.
1/2 sized maintained spell box.
1/2 sized spell effect window (really only use it to make sure I am eating and drinking)
Modified EXP bat with tracking to the 10th percent as well as zone, level, and class.
Quest window is in upper right corner but set to invis until it updates or I mouse over.
EQ button to the right of the compas, set to invis until I mouse over.
Team bar modified for larger clearer health / power bar display.
User window modified to display number and percent as well as bar.
All fonts in game replaced with Arial.
Chat text shrunk on both windows.
RAID group window has TRANSPARENT background (not shown in image below).
 
Overview - I play an inquisitor so alot of my game is watching the health bars of my team members... This arrangment give me all my heals, reactive heals, buffs, debuffs, emergency, and utility spells on the screen.  All users are layed out next to the maintained spell list and the spell hotkeys. I can quickly scan health, what spells have worn off, and recast dely.  I use keyboard shortcuts for 90 percent of my gameplay so the small size does not hinder me.  Clickable spells are usually out of combat.
 
I also re-mapped my group hotkeys away from F1, F2, F3, etc and to 0,1,2,3 etc on the NUMPAD.  This allows me to hit group members with my right hand and key in the spell hotbar with my left.  I have been very happy with this arrangements and have only been making minor tweaks as new mods come out.
 
Many thanks to the crew over at eq2mods.  I have taken so many elements from different sets that I cant name all the authors, if you see you work in the screenshot below know you have my thanks.
 
For a cleric / healer / inquisitor, this is a very effecient UI.

I believe the mod you are looking at is part that I stripped from the "Tablets II" mod posted there, all credit and worship should go to the author of the mod of course.
