getting eq2 client failed

getting the text eq2 client failed when game ends and roster of players and their kills/damage/etc shows up then says microsoft investigating it then eq2 crashes to my windows 7 u.i. Come back and it says I forfeited game. grrrrrr. Also, Another game out of 21 games played today didn't give me any tokens or a.a. even though I played the full game, never left it, was never afk,etc.
