I did the "reinstall eq2" but don't think that includes all expansions.  Please help.

"Reinstall EQ2" should download the patcher. The patcher will be flagged to download anything you buy.

I downloaded the luancher and that downloaded like 9.5 gigs. Friends who biought the same package off of steam got like 13 gigs, so I'm assuming I didnt get the expansions to download.. 

Any idea how to make it download it? I hit optional content and its doing another 2 gigs. I'm really confused here.

did you set the download to pull the optional content? The voice overs and such will pretty much make up the difference
