Dumped to Character Select at end of Trial of Truthbringer

Three times now, at the end of Trial of the Truthbringer, the Marr diety quest, I've gotten a message that I've passed the last trial, but instead of being revived I get kicked to the character select screen, and when I log that character back in I'm outside the instance and my journal says I have to complete the three trials. What might I be doing wrong to make this happen?

Thanks, I will petition. I just wanted to ask first if there was some obvious -- to others but not to me -- mistake I was making.
