Lost recipe for Oak-Shafted Wurmslayer

I'm trying to clear out old quests, and I want to finish a Thorn of Old. I need to forge the re-shafted wurmslayer, but lost the recipe that the old man behind the waterfall gave me. I can't seem to get the recipe back.

Any ideas, short of deleting the quest and starting all over?
