Just a tought to add to the weird screwdriver, could it be possible that its bugging when a pet (be it perma or some dumbfire) gets the killing hit? that could also explain ppl having more luck...

If you do the questline grouped with a friend, and go to the Unveiled palace zone in, it will zone you both into the same instance and tell you 'you have exceeded number of players' and one gets...
