I understand the basics of lunge reversal, but how often does it go off? from the description it looks like it only fires every 20/30 seconds, that doesn't seem to helpful to me. How often does it really go off? could it be usefull vs monks that avoid a lot?

It is set to go off every 20 seconds, though it is in 'cycles' of refreshing itself. Ie: You start a fight when the 'refresh counter' is only 5 seconds away, Lunge Reversal procs in those 5 seconds, then it refreshes again, meaning it can proc again. So you can have 2 procs in any fight that lasts more then a second. You can have three procs in any fight that lasts more then 21 seconds, etc.

Also it sometimes bugs and procs more then once every 20 seconds consistently.

At 8 points in it, it does ~600-800 damage, with crits of up to 1k or so possible. It goes off everytime your opponent dodges/parries/deflects and everytime YOU dodge/parry/deflect someone else's attack.
