Im pretty sure I must be doing something wrong..
But I have my shiriken bag equiped.. i have stars in my ammo slot.
When I target something and hit the ranged attack button I go into a fighting stance and never throw the star.  What am I doing wrong?

Are you in range to use them?  Maybe you are too far away or too close.  What kind of bag do you have?

Ive tried from different ranges, from far away to within aggro range.. no luck.  Umm Im at work so Im trying to remember what I have for a bag.. I think its something like a tanned leather such and such.

There doesn't seem to be any combat animation for shuriken (or it's very subtle).  I have the same setup you have and throwing stars are working for me, but it's very hard to tell that I did anything at all when I use them.  For starters, if you have combat spam turned off open up another chat window to turn it on in.  You'll get spam messages saying if you're out of range or don't have line of sight.  Secondly, you're probably going to miss an awful lot when you first start so don't expect a lot of damage messages.  The easiest way to test if it is working or not is to use them to pull a nonaggressive creature.  If the creature attacks you, the stars are working fine.

I was trying to pull with them actually.  Hasnt been working so I usually just end up running over and booting them in the [Removed for Content]  =)

Examine the bag and make sure you are using the right type of bag for the ammo.  There are different bags for hammers, daggers, stars and axes.

Pretty sure "pouches" are for shiruken. 
 
An addendum to your question:  Do you have to use upgraded material ammo when you up your pouch/bag?  I just bought a prissy boiled leather pouch and am wondering if ye olde iron shiruken will cut it anymore?
