The only two haste items I am aware of are the Ancient Watcher Ring (usable at 32 or 33), which gives 3% haste, and the Flowing Black Silk Sash (usable at 42), which gives 15% haste. There may, of course, be some fabled drops that I am not aware of.

There is also Everburning Flame/Fire, which are haste self-buffs. Everburning Flame is a 32nd level skill.

Haste does stack, though it is believed there is a cap to the effective delay you can get from a weapon with stacked haste buffs.

Item haste stacks. Item haste + spell haste stacks. However our Spider Stance and Everburning haste spell buffs do not stack.

I am pretty sure it does. One monk was talking about having 175% (or so) haste.

Haven't had a chance to check on those but with the MainHud command you can see your attack speed buffs, should be easy to tell if they are stacking. Some people assume all will stack but just because you have multiple buff icons on you does not mean they actually stack, some will overwrite effects of the others. Would have to either parse it or check with the command to see exact figures and tell what all really stacks.

Just got my second slayer's ring and they stack... and then my favorite Chanty cast Alacrity on me and it stacked on top without loss.
