Error On Log Off

<p>Everytime I log off I see this ugly little window:</p><p><a href="http://i55.tinypic.com/aetv8n.jpg">http://i55.tinypic.com/aetv8n.jpg</a></p><p>Could somebody please tell me why and how to fix it?</p>
