I have the +40 to heal spells for the hands adornment.

The question I have is what determines what the "up to..." is?

The amount added is always consistent, when you examine the spell and equip/unequip the piece I was getting about +25 to my heals (ie 950-1250 vs 925-1225). What makes it higher? It doesn't appear to be random, so either there is something that makes it higher, or its deceiving when up to +40 really means +25.

the "up to..." is standardized by cast time etc.

dont know...

I know i added the neck item from acad with symmetry2? it added 1 to the min amount of the tick on my group hot and added nothing to the max of the tick. And thats for my lvl 70 spell.

I think for ticks and dots it / by the number of ticks but of a max of 5. if it is aoe / by another 3 and for other spells it is "normalized" for a 3sec cast
