Poison - Buff Window?

I've had poisons disappearing from my buff window after one fight, not sure whats going on there, only seemed to happen with playermade stuff too, not vendor bought poison

I had something like that happen a few days ago.  Applied a poison, didn't see it in the buff window, equipped a different poison and applied it and that one showed up.  Noticed a bit later after zoning that I now had 2 poisons showing.
